The author of Continuity [Fabric] Mod 1.18.1/1.17.1 built it around modern APIs. It’s feasible for the user to experience the connected textures in the most effective manner.

More information about Continuity
It is a Fabric mod. It’s also a creation that supplies full Optifine parity for every resource pack that controls the Optifine/MCPatcher CTM format.
The mod depends on the Fabric API and it is client-side only. It appears with two resource packs. One offers default connected textures like Optifine. The other gives a fix for glass pane culling.
CTM
At present, there are two major CTM (Connected Texture Mapping) formats.
Optifine/MCPatcher format
Chisel/ConnectedTexturesMod format
The mod only supports the Optifine/MCPatcher format! If it’s necessary to support the Chisel format, please consider using the ConnectedTexturesMod for Fabric!
Remember that deploying the mod and CTMF at a time is an unsupported configuration, at least for the time being!
About Forge Version
The developer doesn’t work on a Forge version due to two reasons:
Optifine. It has native assistance for Forge. Therefore, a Forge version would not be very helpful in the first place.
Forge’s model API. It is greatly different from Fabric’s model APAI (FRAPI), which is controlled by Fabric. Additionally, swapping to it would not only ask for a significant amount of effort. Unfortunately, it can cause the mod to operate inefficiently.
